are member instructions to AssureCare’s secure on-line services offering 24/7
access to your health plan transactions and other information.
Once you have logged in to the member browser
you will have access to detailed information about your claims, including:
Total charges, Discounts, Amount paid, Date
received, Date Processed, Date paid, Coordination of benefits with other plans,
date of services, remarks, and more!

| Step 1 Type www.assurecare.com in the address bar of your browser |

Select the member button located in the on-line services area of AssureCare’s
home page. |

| Step 3 Enter Your health plan “Group Number” located on the front of your ID card |
 |
| Step 4 To initially gain access to our member services browser you will be required to
set up a new account. To begin the new account process select the “click here”
link for members.
(If you have previously set up your account, skip to step 8) |

| Step 5 Fill in the primary insured’s last name
Fill in the primary insured’s social security number. Do not use dashes (example
469747777)
Fill in the primary insured’s date of birth |

| Step 6 You should now be at the edit new member account page. The following information
is required to access your account
- Enter in a username that you can remember. (For example “jdoe”, for John
Doe)
- Enter in a password. For your security we recommended that you use 7 or 8
characters and a combination of letters and numbers. For example “abcd4567”)
- Enter your email address

Your health plan User Name and Password will identify you as the only person
who can access information on your health plan. For this reason, please keep
this information confidential.
The remainder of the fields on the new member account page are optional. When
you are finished click on the submit button.

| Step 7 You have now completed the new member account process. Select “click here” to
return to the login screen. |

| Step 9 You are now ready to begin utilizing AssureCare’s 24/7 on-line health plan
member browser |
